Memorial Hermann Southwest

The Diabetes Self-Management Program at Memorial Hermann Southwest is an American Diabetes Association recognized program having met national standards for Diabetes Patient Education.

Increased patient referrals a commitment to excellence led to an expansion of our program. We now offer ten diabetes management classes per month for type 1, type 2, and gestational diabetes patients. A Saturday class (8:30 - 11:30am), and a weekday evening class (4 - 7pm) are offered once a month for patients unable to attend regularly scheduled classes.

The classes will cover the Principles of Diabetes Management and aim to serve more patients per month. Education in Spanish is also available for Spanish-speaking patients.

Our Staff

Experienced professional counselors include a Registered Nurse and two Registered Licensed Dietitians. All Certified Diabetes Educators are committed to providing patients with the knowledge, skills, and motivation to effectively manage diabetes.

Topics Covered

Diabetes management program topics include: insulin administration/medications, blood glucose monitoring/goals, foot care, complications, and nutritional management - including carbohydrate counting, meal planning, food labels, and dining out.

Individualized follow-up visits are highly recommended to provide feedback after making recommended lifestyle changes.
