After the form is
submitted, a representative from WorkLink will contact your representative
to review protocol and process your specific service requests.
Completing the employer
protocol form establishes your company's policies and protocol for:
- Injury treatment
- Contact information
- Drug-testing
policies
- Billing instructions
- Specific follow-up
with designated company physicians
WorkLink will also
contact the occupational medicine coordinator at the hospital closest
to your workplace to make sure he or she is aware that your protocol
has been established and that it is understood.
Completing
the form also makes your employer protocol available at all the
Memorial Hermann hospitals and clinics.
