Birth Certificate Information
One of the many responsibilities brought by the birth of your newborn is the completion of a birth certificate. The birth certificate is an individual’s only claim and proof of citizenship, identification and relationship to his or her parents. This document serves as the primary document for each of us to enter school, play little league sports, obtain a social security number and account, a driver’s license, a passport and lastly to prove we are citizens and are qualified to work in this country.
It is important to register the birth of your newborn with the Texas Department of State Health Services Vital Statistics as soon as possible. If you would like to get a head start on your baby's birth certificate, you may fill out the Birth Certificate Worksheet form and bring it with you to Family Birth Center when you have your baby.
Should you have any questions, do not hesitate to contact the Birth Certificate Clerk at 281-364-2413. Please remember to bring your completed forms with you to the Family Birth Center when you have your baby. You may also fax the Birth Certificate Worksheet form to 281-364-4197 or e-mail to Aimee Evins. The Birth Registrar will submit this information to the Vital Statistics before your discharge.
Learn more about completing the birth certificate, obtaining a certified copy of a birth certificate, or establishing paternity if you are unmarried, or not married to the biological father.
Information for Unmarried Parents
Please note that Form 1608 Acknowledgment of Paternity is voluntary. If you are not married to the biological father of your child but you want his name on the birth certificate, the Acknowledgement of Paternity (AOP) form must be completed. If you are married to someone other than the biological father of the child, or have not been divorced more than 300 days your husband/ex-husband must complete the Denial of Paternity (DOP) section of the AOP. If the husband/ex-husband does not complete and sign the DOP, you cannot proceed with the AOP. Visit the Birth Certificate Clerk at Memorial Hermann The Woodlands Hospital to obtain information about the AOP form and instructions for completion.
Acknowledging paternity for a child born to unmarried parents is very important. Learn more about establishing paternity at the Web site for the Attorney General of Texas.

4D Ultrasound
This ultrasound capability allows expectant parents to share high-quality images of their infants before birth. The 4D ultrasound system is used to capture photos of babies at 28 – 30 weeks. While diagnostic imaging studies ordered by physicians seek primarily to assess the baby’s well-being, 4D ultrasound produces keepsake pictures that highlight facial features and movements. The new service requires a physician referral and is not covered by insurance. The $150 fee is payable by cash, check or credit card. For more information or to schedule an ultrasound, please call 281-364-2514.
