Diabetes Self-Management
The goal of the Diabetes Self-Management Program is to provide patients with the knowledge, skills and motivation needed in order to effectively self-manage their diabetes. Effective self-management helps to minimize, delay or avoid complications associated with the long-term disease process. Memorial Hermann Katy Hospital has been Recognized by the American Diabetes Association for Quality Self-Management Education*. Our Diabetes Self-Management Program has met the National Standards for Diabetes Self-Management Education.

This program is for people with all types of diabetes: type 1 diabetes, type 2 diabetes, gestational diabetes, newly diagnosed, and well-established patients that want to improve their blood glucose control.
An individualized approach, that may include a group setting, allows the patient to identify the behavior changes needed to control diabetes. Diabetes Education Services is staffed by instructors specializing in diabetes management.
A Physician’s Referral is required. Instruction in Spanish language is available upon request.
Additional Support
Memorial Hermann Katy offers a Diabetes Support Group for all those affected by diabetes mellitus. The group meets quarterly in the evenings.
