The Texas Kidney Institute office hours are Monday through Friday, 7:30 a.m. to 4:30 p.m.

Please provide the following information when scheduling:

• Referring physician name and address
• Insurance information
• Authorization or referral.
• Faxed copy of the patient’s insurance card (front and back)
• Some procedures will require physician order, a current history and physical exam
